After ten years of serving Southern Nevada, Sandstone Psychological Practice has officially closed. If you are a former patient and need access to your medical records, please complete the request form below or contact Dr. Mihelic at (702) 900-7906.

Most requests for records will be completed within 24-48 hours; however, if you are asking for clinical information that requires that your previous provider to be contacted, we may need additional time.

You may also search for your former clinician to see where they are currently providing services and reach out to them directly for any clinical or record requests needs.
